Senior Information Services/Caregiver Advisor(SIS)

Senior Information Services (SIS) provides a variety of services to older adults and thier caregivers as well as the disabled.
The Coles County Council on Aging (CCCoA) sponsored SIS program is the designated Coordinated Point of Entry for Information, Assistance, and referral Services (CPoE) for the Coles County area. Meaning, we are the starting point for such services. 
Our team utilizes a "person-centered" planning approach in regards to the information, assistance, and referral services that we provide.
Services include and may not be limited to:
  • Benefits Counseling
  • Long Term Support Options Counseling
  • Assistance or Completion of Forms and Applications

              •Benefits Access Applications

              •Medicare enrollment

              •Part D Prescription Plan

              •Medicaid

              •Homestead Exemption

              •Referrals to Other Services Provided by Other Agencies   

  •     Caregiver Advisory Services (CG) or Grandparents Raising Grandchildren(GRG)

               •Information Resource Assistance

               •Counseling

               •Support Groups

                •Training

                •Respite

                •Referrals
